Gave భూతకాలం కాబట్టి was + V3 (given). రెండు Objects ఉన్నప్పుడు వ్యక్తి Object (me) ను Subject చేయడం సాధారణం: I was given a book by him. "am given" వర్తమానం, "was gave" లో V2 వాడటం తప్పు; "A book gave me" లో క్రియ రూపం తప్పు.

Correct: C — The form had to be signed by her.

had to తర్వాత కూడా be + V3 వస్తుంది, మరియు had to భూతకాలం కాబట్టి had to అలాగే ఉంటుంది: had to be signed. "has to" వర్తమానం అవుతుంది; "had been signed" అర్థం మారుస్తుంది (బాధ్యత కాదు, పూర్తయిన పని).
